Grace Chiodo
I have over 30 years of experience in the mental health and disability sector, supporting people to live meaningful and fulfilling lives. Throughout my career, I have worked in a variety of roles, including ABI Case Management, Intake and Assessment, Mental Health Team Leadership, Respite Coordination, Carer Support, Community Support, and, most recently, NDIS Support Coordination.
I am passionate about working alongside people living with mental health challenges, focusing on their strengths, recovery, and individual goals. I believe there is no “one size fits all” approach, and I enjoy thinking outside the box to find creative, person-centred solutions that meet each person’s unique needs.
After many years working within organisational systems, I became frustrated by restrictive policies and rigid approaches that often-limited opportunities for people to achieve their goals. Driven by my values and commitment to compassionate, flexible support, I established my own practice so I could put people first and tailor support to the individual, not the system.

Sephora Kita
I hold a Bachelor of International Relations and have over seven years of experience in the disability sector, supporting individuals and families to navigate the NDIS and achieve meaningful outcomes. As a Support Coordinator, I am passionate about empowering people to achieve their goals, build independence, and access the supports they need to live fulfilling lives.
I take a person-centred approach, focusing on each individual’s unique strengths, needs, and aspirations. By building strong relationships based on trust, respect, empathy, and open communication, I help participants and their families feel confident and supported throughout their NDIS journey.
My professional background, combined with my genuine compassion for people, enables me to effectively advocate for participants and connect them with the right services, community opportunities, and resources to help them achieve their goals. Whether coordinating supports, strengthening community connections, or building participants’ capacity and independence, I am committed to providing practical guidance and making a positive difference in the lives of those I support.

Michelle Hessey
Supporting people isn’t just my job – It’s something I genuinely care about. As a Support Coordinator, I am passionate about helping participants navigate the NDIS with confidence, overcome challenges, and build lives that reflect their own goals, choices, and aspirations
I believe every person deserves to feel heard, respected, and empowered. My approach is centred around building genuine relationships based on trust, honesty and compassion. I take the time to understand each participant’s unique circumstances so I can connect them with the right supports and advocate for the services they need to live the life they choose.
I have a particular passion for working alongside people with complex needs, including those experiencing mental health challenges and housing instability. I enjoy thinking creatively, solving problems, and working collaboratively with participant’s, families, and providers to achieve positive, lasting outcomes.
Being a Support Coordinator is incredibly rewarding because I get to witness people grow in confidence, gain independence, and achieve goals they may not have thought possible. Whether it’s securing the right supports, advocating during difficult times, or celebrating the small wins along the way, I am committed to walking alongside participants every step of their journey.
For me, this role is about more than coordinating services-it’s about making a genuine difference in people’s lives through kindness, advocacy, dedication, and unwavering support.

I am a dedicated Support Coordinator who is passionate about empowering participants to achieve greater independence, wellbeing, and quality of life through person-centred support and strong advocacy. I have experience supporting participants with complex psychosocial, mental health, neurological, and physical support needs across a wide range of NDIS services.
I work closely with participants, families, allied health professionals, and service providers to ensure supports are effectively coordinated and aligned with each participant’s individual goals, preferences, and capacity-building needs. Known for my calm, approachable, and solution-focused approach, I am committed to helping participants navigate the NDIS with confidence while building sustainable support networks within their communities.
I value collaboration, clear communication, and culturally responsive practice, ensuring each participant feels heard, respected, and supported throughout their journey. I am particularly passionate about helping participants access meaningful community participation opportunities, improve emotional wellbeing, build independence, and explore long-term pathways for housing, employment, and social inclusion.

I am an experienced NDIS Support Coordinator and Team Leader with over five years of experience in the disability and community services sector. Holding a Master of Business Administration (MBA) and qualifications in Public Health, Individual Support, and Community Services, I bring a strong combination of leadership, strategic thinking, and a passion for helping people achieve their goals.
I am committed to delivering person-centred, strengths-based support that empowers participants to exercise choice and control while navigating the NDIS with confidence. I have experience supporting individuals with autism, psychosocial and mental health conditions, intellectual and physical disabilities, complex support needs, and housing pathways.
Working collaboratively with participants, families, carers, and service providers, I focus on coordinating supports, building capacity, advocating for positive outcomes, and helping individuals achieve greater independence, community participation, and quality of life. I believe every participant deserves to feel heard, respected, and supported to achieve their goals and live the life they choose.
Ming Jian Hor
I am a Psychosocial Recovery Coach and Support Coordinator with experience supporting people living with mental health challenges, complex psychosocial needs, and a wide range of disabilities. I have worked alongside people from diverse cultural backgrounds, as well as individuals with physical disabilities including those who are blind or have vision impairment, people who use wheelchairs, and people who are Deaf, hard of hearing, or non-verbal. I work collaboratively with participants to navigate the NDIS, coordinate the right supports, and build the skills and confidence needed to live more independent, meaningful, and self-directed lives.
Working collaboratively with participants, families, carers, and service providers, I focus on coordinating supports, building capacity, advocating for positive outcomes, and helping individuals achieve greater independence, community participation, and quality of life. I have particular experience working with people managing mental health challenges, substance use recovery, housing and environmental stressors, and complex system navigation needs across a range of disability types and cultural contexts.
