Grace Chiodo

DIRECTOR

I have over 30 years of experience in the mental health and disability sector, supporting people to live meaningful and fulfilling lives. Throughout my career, I have worked in a variety of roles, including ABI Case Management, Intake and Assessment, Mental Health Team Leadership, Respite Coordination, Carer Support, Community Support, and, most recently, NDIS Support Coordination.

I am passionate about working alongside people living with mental health challenges, focusing on their strengths, recovery, and individual goals. I believe there is no “one size fits all” approach, and I enjoy thinking outside the box to find creative, person-centred solutions that meet each person’s unique needs.

NDIS SUPPORT CORDINATOR

I hold a Bachelor of International Relations and have over seven years of experience in the disability sector, supporting individuals and families to navigate the NDIS and achieve meaningful outcomes. As a Support Coordinator, I am passionate about empowering people to achieve their goals, build independence, and access the supports they need to live fulfilling lives.

I take a person-centred approach, focusing on each individual’s unique strengths, needs, and aspirations. By building strong relationships based on trust, respect, empathy, and open communication, I help participants and their families feel confident and supported throughout their NDIS journey.

NDIS SUPPORT CORDINATOR

Supporting people isn’t just my job – It’s something I genuinely care about.  As a Support Coordinator, I am passionate about helping participants navigate the NDIS with confidence, overcome challenges, and build lives that reflect their own goals, choices, and aspirations

I believe every person deserves to feel heard, respected, and empowered.  My approach is centred around building genuine relationships based on trust, honesty and compassion.  I take the time to understand each participant’s unique circumstances so I can connect them with the right supports and advocate for the services they need to live the life they choose.

NDIS SUPPORT CORDINATOR

I am a dedicated Support Coordinator who is passionate about empowering participants to achieve greater independence, wellbeing, and quality of life through person-centred support and strong advocacy. I have experience supporting participants with complex psychosocial, mental health, neurological, and physical support needs across a wide range of NDIS services.

I work closely with participants, families, allied health professionals, and service providers to ensure supports are effectively coordinated and aligned with each participant’s individual goals, preferences, and capacity-building needs. Known for my calm, approachable, and solution-focused approach, I am committed to helping participants navigate the NDIS with confidence while building sustainable support networks within their communities.

NDIS SUPPORT CORDINATOR

I am an experienced NDIS Support Coordinator and Team Leader with over five years of experience in the disability and community services sector. Holding a Master of Business Administration (MBA) and qualifications in Public Health, Individual Support, and Community Services, I bring a strong combination of leadership, strategic thinking, and a passion for helping people achieve their goals.

I am committed to delivering person-centred, strengths-based support that empowers participants to exercise choice and control while navigating the NDIS with confidence. I have experience supporting individuals with autism, psychosocial and mental health conditions, intellectual and physical disabilities, complex support needs, and housing pathways.

Ming Jian Hor

PSYCHOSOCIAL RECOVERY COACH & SUPPORT COORDINATOR

I am a Psychosocial Recovery Coach and Support Coordinator with experience supporting people living with mental health challenges, complex psychosocial needs, and a wide range of disabilities. I have worked alongside people from diverse cultural backgrounds, as well as individuals with physical disabilities including those who are blind or have vision impairment, people who use wheelchairs, and people who are Deaf, hard of hearing, or non-verbal. I work collaboratively with participants to navigate the NDIS, coordinate the right supports, and build the skills and confidence needed to live more independent, meaningful, and self-directed lives.
